Understanding the Role of Advanced Practice Providers and the Team Care Approach

At our rheumatology practice, we are committed to providing the highest quality of care through a collaborative team approach. We aim to teach our patients about the important roles played by Advanced Practice Providers (APPs) and how they work together with our rheumatologist to ensure that you receive comprehensive and personalized medical care.

Who Are Advanced Practice Providers (APPs)?

APP's are highly trained healthcare professionals who are integral members of our medical team. Advanced Practice Providers are known as Physician Assistants or Advanced Practice Registered Nurse. Our APRN’s are registered nurses with advance education and clinical training, to provide a wide range of preventive and acute health care services to individuals of all ages. They have complete graduate-level education preparation that leads to a master’s or doctoral degree. They take health histories and provide complete physical examinations; diagnose and treat many common acute and chronic problems; interpret laboratory results and imaging studies; prescribe and manage medications and other therapies; provide health teaching and supportive counseling with an emphasis on prevention of illness and health maintenance; and refer patients to other health professionals as needed.

In Our Practice, We are Proud to have:

- Carlos Sanchez, APRN & Arnol Chacon, APRN-Two nurse practitioners who are international medical doctors in their respective countries. They bring a wealth of medical expertise to their roles.
- Dr. Ernestina Kyei Donkor, DNP APRN is a Nurse Practitioner with a Doctorate in Nursing Practice (DNP). She brings extensive knowledge and advanced skills in patient care
- Carlos Sanchez, our Nurse Practitioner, is a trained international medical rheumatologist from Cuba, providing specialized insight and expertise in rheumatologic conditions.

The Benefits of Team-Based Care:

1. Comprehensive Care: Our team approach ensures that all aspects of your rheumatologic health are addressed. Our APP's work alongside our rheumatologist to provide a well-rounded perspective on your health needs.
2. Increased Access to Care: With three Advance Practice Providers, patients can often get appointments sooner, reducing wait times and ensuring timely medical attention.
3. Personalized Attention: APPs often spend more time with patients, offering detailed explanations and answering questions to ensure you understand your health condition and treatment options.
4. Collaborative Treatment Plans: By working together, APPs and our rheumatologist develop comprehensive treatment plans tailored to your specific needs, enhancing the quality and continuity of care.

How We Work Together:

- Initial Consultation: You may start your visit with one of our APPs who will conduct a thorough examination and gather your medical history.
- Collaborative Review: APPs consult with our rheumatologist to review your case, discuss findings, and determine the best course of action.
- Treatment and Follow-Up: Depending on your needs, you might see either an APP or our rheumatologist for follow-up visits. Both are qualified to manage your care, prescribe treatments, and adjust plans as necessary.
